GENERAL PHYSICAL EXAMINATION- 

Vitals- 
Heart rate- 86bpm
Respiratory rate- 30cpm 
Temperature- 102 F
Blood pressure- 130/70 mmHg 

Mild pallor present 

No signs of Cyanosis, clubbing, edema, lymphadenopathy 



Systemic Examination-

Cardiovascular examination- 
JVP raised, Apical impulse seen on 5th left intercostal space 
S1S2 heard, Apex beat is felt at 5th intercostal space 1.5cm  lateral to mid clavicular line 


Diagnosis- CKD on MHD
