1) UNIT POSTINGS:-

I was posted into Unit 2. For the first two weeks we were under the guidance of Dr Nikita ma’am (SR), Dr. Pavan sir (PGY2) and Dr.Lohith Sir (PGY1). The last two weeks we were under the guidance of Dr.Nikita ma’am (SR), Dr Hari Priya ma’am (PGY2) and Dr Govardini ma’am (PGY1).
My first day into the department, our unit had our first OP day.
OUT-PATIENTS BLOCK:
- Checking vitals for OP patients in an organized manner so it will be easier to take history and prescribe the required medications if necessary.
- Took history and proper examination for OP patients along with proper counseling
- Understood how to investigate admitted cases along with proper management for the same.
CASUALTY NIGHT DUTIES:
- Understanding important investigations to be sent for acute cases along with Quick collection of reports for emergency cases to help ensure even more precise treatment for the patients.
REGULAR UNIT DUTIES:
- created PaJR groups for every patient admitted within the unit.
- Blogs of patients for PaJR groups after taking consent and de-identifying.

- updated SOAP notes daily for respective patients.
- Wrote and updated fever charts along with updating de-identified investigations for PaJR groups.
- Collection of reports and made sure it’s all updated.
- Understood ABG interpretation along with management.
- Getting investigations done for admitted patients in the unit from ward, ICU and AMC.
- Taught basics of Respiratory examination to 5th semester UG students.
- Explained abdominal examination to UG students, namely how to elicit shifting dullness and fluid thrill.
- special mention of a incidental finding of Dupuytren’s contracture in a patient admitted for ascites.
2) PERIPHERAL DUTIES:-

A) Ward
Performed the following:
- Foley’s catether- for 4 patients
- Performed therapeutic Ascitic tap in a patient with Chronic Liver disease (1-1.5 L of Ascitic fluid)
- Drawing venous blood for sample collection for investigations for 7 patients
- IV cannula insertion for 3 patients
- Monitoring vitals of patients
- Teaching UG students on how to elicit knee reflex in a patient with CVA.
- Hourly (and half an hrly if required) vitals monitoring for patients undergoing dialysis
- Understood the importance of vitals Monitoring and how it makes a difference along with aiding in the process of dialysis
- Helping with discharges along with updating vitals and investigation reports of patients admitted under nephrology.
- Diagnostic Pleural tap done on a CKD patient with gross pleural effusion under the guidance of Dr Navya ma’am and the pulmonology team.
- ABG sample collection from Radial and femoral arteries for 3 patients.

3) PSYCHIATRY POSTINGS:-
~ from 13/07/2023- 27/07/2023
- Attended psychiatry OP cases, and understood a spectrum of psychiatry disorders in various patients ranging from personality disorders, to anxiety disorders etc.
- Night duties: for emergencies and monitoring certain cases such as withdrawal syndrome and understood how to manage them.
- Understood ICD-10 and DSM classification
- Typed discharge summaries for various de-addiction patients and attended their counseling sessions as well.
- Had end posting exam where we were able to review what all we had learnt from our posting as well as a recap of theory and pharmacological treatment for the same.
